Outreach Services & Support
All PBH Outreach programs are staffed by individuals with lived experience who have navigated similar challenges in their lives. PBH outreach programs support individuals in accessing supportive services and navigating systems of care.
Service connections can include housing resources, benefits advocacy, mental health care, physical health care, substance use treatment, disability support, and other services to enable individuals to move toward their goals.
The Outreach Department staff operate in the community through street-based outreach as well as intensive and light case management services. We aim to work as a community to help people work towards resiliency, recovery, and lead a self-directed life.
The Outreach Department has 3 programs:
1. R.E.A.L. (Recovery, Empowerment, Advocacy, and Linkage) – Works with individuals who have substance use disorder and Mental Health conditions who may have frequent contact with Law Enforcement or the Justice System.
2. PATH (Project for Assistance in Transition from Homelessness) – Works with individuals who are experiencing Serious Mental Illness or co-occurring disorder, may be homeless or at imminent risk of homelessness, and may also be a Veteran.
3. Peer Pathfinder (A branch of the PATH program) – Works with individuals who are experiencing Opioid or Stimulant Use Disorder, may be at risk of an overdose and may also be frequent utilizers of the Emergency Department.

Who Can Access These Services?
Individuals do not need to be a PBH Client to receive assistance from any of the outreach programs.

How we can help
Evaluate your individual needs and barriers to housing.
Develop an individual service plan based on your specific needs.
Assist in applying for and obtaining benefits.
Provide emotional support and understanding.
Connect to primary and behavioral health care, dental, and chemical dependency services.
Facilitate access to basic services such as food, shelter, and hygiene centers.
Assist in connecting with transportation, vocational training, and other services.
Support you as an individual as well as your family as a whole.
What defines homelessness?
A homeless person is someone who lacks a regular, adequate nighttime residence. People who are homeless or facing homelessness may be living outdoors, on the streets, or in a car. They may be residing in a shelter or other temporary housing, or they may be staying in an unsafe, inhabitable space. PATH services are available for those who are currently homeless and those at risk of becoming homeless. You do not have to be a PBH client to benefit from PATH.
